Preparing funeral arrangements for your loved one is never an easy task. Here are some things to consider as you start the funeral service process.
Death Certificate:
You will need to fill out and complete a death certificate form. To do so you will need to have your loved ones legal name, social security number, age, date of birth, birthplace, address including zip code, marital status at time of death, surviving spouse name (if wife maiden name), fathers name, mothers name (maiden name), race, education, occupation (work done most of living life), type of industry, method of disposition, place of disposition, location, doctors name, address, phone number, place of death, address of location, date of death and time of death.
Deciding on an appropriate Service time:
Considering, your loved ones time frame, drive time, minister, church availability, cemetery location and availability.
Obituary information:
We will begin with your loved ones name, nickname, age, residence, church membership, work place, special interest such as hobbies, associations, special affiliations and military service. Make a predeceased list of your loved ones who have passed away. Followed by the survivors (parents, sons, daughters, grandchildren, great grandchildren, sisters and brothers)
Which newspapers you would like your obituary to be placed in.
With or without a picture.
The funeral home will check on placement time and cost.
Military
Was your loved one a veteran? Retired?
If they were honorably discharged we will need to have a copy of his or her DD214.
Who will be presented with the flag?
Signing and filing for military benefits.
Signing and discussing your options on the veteran’s marker.
(The government only provides the marker. This does not include any setting fees, cemetery charges, or the concrete pad.)
Memorial Package:
Selecting the memorial package that best suits your loved one.
Collecting information for inside: you will be selecting a verse or poem.
Selecting music to be played during the service.
List of your pallbearers and honorary pallbearers.
Selecting a casket:
Sealing or Non-sealing
Gauge
Color

Concrete grave liner
Is considered non-protective-it does not seal. It is referred to as a rough concrete box.
Burial Vault
Is considered protective-it is designed to seal once the casket is placed inside the vault. It houses, shelters and protects the casket and the deceased from the entrance of water, soil and other ground elements.

Would your family like to have a family car?
A family car is an additional charge. We will arrange for it to take you to and from the service.
Escorts needed?
We are required to order an escort to follow in precession to the cemetery.
Honorariums?
A gift given to your officiating minister.
Death Certificates?
How many certified death certificates are needed?
There is a fee. This fee is charged by the state.
Would your family like to have a DVD memorial tribute made?
